Patient Online Forms
Upon scheduling an appointment as a new patient in our office, you will be asked to provide an email address. Our receptionist will forward information on what needs to be done to complete the online forms. They provide us with general information about yourself and your condition. We will request you fill out the patient forms prior to your initial visit, allowing the doctor to look over your concerns and be aware of the topics and situations you are interested in resolving.
Next you will have a consultation with the doctor to discuss your health-related concerns, and gather more information about your concerns. Your first visit is designed for the doctor to learn more about you, your condition and your expectations so that they may to determine whether chiropractic care will you meet your goals and what options are available.
After your consultation your doctor will perform a complete chiropractic examination testing your reflexes, your ability to turn and bend as well as other standard neuro-logic, orthopedic, postural, and physical examinations. Nothing will be done in our office without your consent.
Your specific condition may require us to take x-rays to to get a full evaluation of a patient. If needed you will be sent to one of our x-ray/ MRI facilities. X-rays help us rule out more serious conditions and assist us in developing the most effective treatment plan for you.
Patients will generally be provided with their first treatment at the conclusion of the initial appointment. This may include spinal adjustments, physical therapies, and/or soft tissue therapies. Treatment is interactive with you so you can express any concerns you have about the style of adjustment you will be most comfortable with.
Report of Findings
On your second visit to the office, once the information has been collected and the examinations have been performed, the doctor will give you a Report of Findings booklet that is specific to your needs. If you have a condition that requires care with other providers, the doctor will tell you. If the doctor believes we can help you, he will recommend treatment options and a wellness program. This booklet will help give directions, as well as answer questions such as:
1. Can you help me?
2. What do you recommend that I do to get better?
3. What are things I can do to improve my health?
Prior to leaving, patients will be instructed on a wellness program to be conducted at home. If you are in pain when you first come into our office this may include ice or heat application instructions, avoidance of certain activities or positions, as well as home exercises and/or stretches. If you desire an overall wellness lifestyle, our wellness team will work with you to create good habits and routines that support your success. Every body is unique and different and therefore everyone requires a unique and customized wellness plan. The foundation of our overall wellness lifestyle is maintaining healthy posture and good spinal alignment, eating a healthy diet, taking supplements, keeping your weight under control and stress relief.
